How we evaluated these apps

This list is narrower than a general mental-health-app list: we focused specifically on conversational AI apps that function as therapy-style chatbots. We excluded meditation apps (Calm, Headspace, Insight Timer), pure mood-trackers (Daylio, Moodfit), and journaling apps without an AI therapy loop — those belong in other roundups. Everything here is an app you actively *talk to*.

Five evaluation dimensions: conversational depth (can the AI handle off-script topics?), therapeutic framework (CBT, DBT, open-ended, other), confrontation vs. validation balance (does it push back or only mirror?), crisis handling (what happens at distress signals?), and pricing transparency.

Each app tested for at least 21 days against five seeded scenarios: everyday anxiety, a specific avoidance pattern (procrastinating a hard conversation), sleep-disrupting rumination, grief-adjacent low mood, and a hypothetical crisis disclosure. Per-app notes call out strengths and failure modes at each.

Honest disclosure: ILTY is published by the authors of this list. We've placed ourselves at #1 for the specific scenario of 'conversational AI with a tough-love option.' For broader scenarios, other apps rank higher — Wysa for clinically-researched CBT, Pi for free conversational patience, Self Help Doctor for structured CBT programs.

Disclosure

Published by ILTY. ILTY is one of the apps reviewed and is positioned by us at the top for 'conversational AI therapy with multiple companion personalities.' Pricing and features accurate as of April 19, 2026, drawn from public App Store/Play Store listings and our per-app reviews at /compare/. We update this roundup quarterly. For the broader category (including meditation and mood-tracking), see /best/ai-mental-health-apps-2026. Update, August 13, 2026: this list was audited for discontinued products after two apps were found still being recommended sitewide after they had shut down. Corrections applied here are marked in the entries themselves. Note the limit of this pass: it looked for dead and dying products, and did not re-verify pricing, ratings, or hands-on impressions for the apps that are still live — those still date from the original testing round.

The research behind this

Rankings on this page draw on The 2026 State of AI Mental Health Apps, our own study of 15 apps across 8 criteria. Crisis handling was tested by hand: the same four escalating messages typed into every app, screenshots kept. Four of them never produced a crisis number. Every score is published with the reasoning behind it, including ILTY’s, and the rubric is CC-BY if you would rather score apps yourself.

The apps, in detail

#1 · ILTY

Best for: Conversational AI therapy with a tough-love option

Conversational AI therapy with five distinct companion personalities.

ILTY is built for the moment when you want real conversation instead of scripted exercises. Mr. Relentless is the differentiator — most AI therapy apps default to validation; ILTY's lead companion challenges avoidance. Best for users who've bounced off gentler AI therapy apps because they felt too soft.

Strengths

  • +Five companions — Mr. Relentless (tough love), Stoic Advisor, The Architect, Mindful Guide, Ember — pick the voice that fits the moment
  • +Conversational AI, not scripted CBT decision trees — handles off-script topics naturally
  • +Every session ends with a concrete action step, not a motivational quote or reframe
  • +Explicit 'no toxic positivity' design — no daily affirmation notifications, no forced gratitude

Where it falls short

  • −Subscription gate after onboarding — no free indefinite tier
  • −Younger product — no peer-reviewed clinical studies yet

Pricing

Free download. Subscription after onboarding: $12.99/mo or $99.99/yr (1-week free trial).

Platforms

iOS (iPhone, iPad)

Free to download · 7-day trial · then $12.99/mo or $99.99/yr

#2 · Wysa

Best for: Clinically-researched CBT conversation

CBT-based AI chatbot with optional human-coach upgrade — the most clinically-researched AI therapy app.

Wysa is the closest thing the AI therapy app category has to a research-backed default. For users who want gentle CBT with clinical pedigree, start here. Especially strong if you want the option to eventually talk to a human coach.

Strengths

  • +Strongest peer-reviewed research base in the category
  • +Genuinely useful free tier
  • +Optional upgrade to a real human coach — bridges AI and therapy
  • +Cross-platform

Where it falls short

  • −Default tone is gentle — can feel slow if you want directness
  • −Free tier pushes hard toward Premium
  • −Penguin avatar is divisive

Pricing

Free tier + ~$99/yr Premium.

Platforms

iOS, Android

vs. ILTY

Wysa for gentle, research-backed CBT. ILTY for when gentle stops working and you want directness.

#3 · Pi (Inflection AI)

Best for: Free, patient AI conversation without therapy framework

Free conversational AI built for emotional listening — not therapy-structured, but therapy-adjacent.

Pi is the free option worth knowing about for pure conversational support without a therapeutic framework. If what you want is 'feels heard,' Pi is often closer to that than a CBT app is. Treat it as conversation, not therapy.

Strengths

  • +Free — no subscription, no in-app purchases
  • +Genuinely patient, empathetic conversational style
  • +Available everywhere (web, SMS, WhatsApp)
  • +No therapy positioning — honest about what it is

Where it falls short

  • −Not CBT-structured — won't teach you skills
  • −No mood tracking, no tools, no frameworks
  • −Inflection AI was acquired by Microsoft in 2024 — long-term direction unclear

Pricing

Free.

Platforms

Web, iOS, WhatsApp, SMS

vs. ILTY

Pi for free, non-structured emotional listening. ILTY for conversations with structure, personality, and end-of-session action steps.

#4 · Youper (SHUTTING DOWN Sept 2026)

Best for: (Used to be: short daily check-ins + pattern recognition)

Emotion-AI with short CBT check-ins and mood analytics — closing September 30, 2026.

Kept as a historical entry because people still search for Youper by name. It was one of the few apps in this category with peer-reviewed research and a 510(k) clearance behind it — it earned its place on lists like this. It closes September 30, 2026, is already delisted from both stores, and deletes user data October 1. For the emotion-tracking-and-reflection job it did, Letheia is the closest living option on this list (free, emotion-wheel-driven, reflection over conversation); Wysa is closer if the short structured CBT session was what you came for.

Strengths

  • +(Historical) Best mood-analytics of the category — emotion patterns over time
  • +(Historical) Short 3-7 minute sessions fit busy schedules
  • +(Historical) CBT + Acceptance and Commitment Therapy foundations, with peer-reviewed publications
  • +(Historical) Cross-platform

Where it falls short

  • −SHUTTING DOWN September 30, 2026 — verified 2026-08-13: the US App Store lookup returns no result, the Google Play listing 404s, and youper.ai carries its own shutdown banner with data-export instructions. Subscriptions are disabled and it is already delisted in the US, UK, Canada, Australia and Brazil. User data is purged on 2026-10-01 — export your history before that date.
  • −(Historical) Yearly-only subscription (no monthly, no free tier)
  • −(Historical) More form-filling, less free-flowing conversation than Wysa or ILTY
  • −(Historical) Emotion detection could feel mechanical

Pricing

(No longer sold — subscriptions disabled ahead of the shutdown)

Platforms

(Delisted from both app stores; app stops working 2026-09-30)

vs. ILTY

Not a live comparison anymore. Letheia or Wysa are the surviving picks for structured mood data; ILTY is for conversations about what's behind the mood, which was never what Youper optimized for.

#5 · Ash by Slingshot AI

Best for: Voice-based AI therapy conversations

Voice-first AI 'therapist' — longer, deeper, more natural-feeling conversations.

Ash is the voice-first option. If text conversations feel laborious and you prefer talking, Ash's voice loop is genuinely different. Strong for users who've bounced off text chatbots.

Strengths

  • +Voice mode as primary interaction — distinct in the category
  • +Conversations feel longer and more exploratory than competitors
  • +Marketed explicitly as 'AI for mental health' with a focused design
  • +Active development, strong early reviews

Where it falls short

  • −iOS only
  • −Less clinical research backing than Wysa or historical Woebot
  • −British Psychological Society has raised concerns about AI 'therapist' framing in general

Pricing

Free trial + subscription for full access (check current pricing).

Platforms

iOS

vs. ILTY

Ash for voice. ILTY for text conversations with personality range. Different modalities for different preferences.

#6 · Letheia

Best for: Emotionally honest mood tracking + reflection

Emotionally honest mood-reflection app with AI chat, breathwork, and games.

Letheia is worth knowing about for the 'no toxic positivity' positioning and the free tier. The AI chat is less core to the product than in Wysa or ILTY — if conversation is what you want, Letheia is a thinner version of that.

Strengths

  • +Free with no paywall
  • +Strong feature breadth: Emotion Wheel, Truth Threads, breathwork, games, Apple Watch
  • +TruthSafe™ patent-pending safety layer
  • +Guided by licensed counselors per marketing

Where it falls short

  • −AI chat is one feature among many, not the core product
  • −Single AI voice — no personality range
  • −Less focused on therapy-style conversation than other apps here

Pricing

Free (no subscription listed).

Platforms

iOS (iPhone, iPad, Mac, Apple Watch, Apple Vision Pro), Android

vs. ILTY

Letheia for a free, feature-broad reflection app. ILTY for conversation-first AI therapy with personality range.

#7 · Self Help Doctor

Best for: Structured CBT as a deliberate program

Structured 12-week CBT program with AI guidance — built by a clinical psychologist.

Self Help Doctor is for users who want the CBT substance without a chatbot UX layer. If you liked Woebot's exercises but found the chat wrapper frustrating, this is the purist alternative.

Strengths

  • +12-week structured CBT course — closer to doing the clinical work than chat
  • +Built and designed by a clinical psychologist
  • +One-time payment model (no subscription)
  • +No AI 'therapist' framing — honest about being self-help

Where it falls short

  • −Web only — no native app
  • −Less conversational, more course-like
  • −Requires commitment to a multi-week program, not as-needed chat

Pricing

One-time payment (no subscription).

Platforms

Web

vs. ILTY

Self Help Doctor for structured CBT as a program. ILTY for ad-hoc conversation with companion personalities.

#8 · Abby.gg

Best for: Free voice AI without downloading an app

Free web-based AI therapy chatbot with text and voice modes.

Abby is worth knowing about as a free voice option. Treat as a light supplement, not a primary AI therapy tool — the product is early and the research backing is thin.

Strengths

  • +Free tier is usable
  • +Voice mode available
  • +No download required (web)
  • +Open-ended conversational style

Where it falls short

  • −Web only — no native iOS or Android app
  • −Younger product, less mature than Wysa
  • −No published clinical research

Pricing

Free tier + paid premium.

Platforms

Web

vs. ILTY

Abby for free web voice chat. ILTY for native iOS conversational AI with personality range.

ILTY is on iOS and Android. Free to download, 7-day trial, then $12.99/mo or $99.99/yr. Try it for the tough-love-processing scenario specifically — and see if the conversation feels different from the apps you've tried.

Frequently asked questions

What's the difference between an AI therapy app and an AI mental health app?

'Mental health app' is broader — includes meditation (Calm, Headspace), mood tracking (Daylio, Finch), journaling, and therapy-style chatbots all in one category. 'AI therapy app' is narrower — specifically conversational AI designed to function as a therapy-style chatbot. This list is the narrower one. For the broader category, see /best/ai-mental-health-apps-2026.

What is the best AI therapy app in 2026?

Depends on what you need. Most research backing: Wysa. Free: Pi or Wysa's free tier. Tough-love conversation: ILTY. Voice-based: Ash. Most structured CBT: Self Help Doctor. Short daily check-ins with mood reflection: Letheia — Youper held this slot until it announced it's shutting down on September 30, 2026. None are a replacement for therapy with a licensed human clinician.

What's the best free AI therapy app?

Pi (Inflection AI) is fully free conversational AI. Wysa has a usable free tier with CBT tools. Letheia is free with no subscription listed. 7 Cups (not on this list — it's peer-support, not AI) has free human peer listeners. None of these replace therapy but all are genuinely usable at no cost.

Can AI therapy apps replace a real therapist?

No. Every legitimate AI therapy app explicitly states this, including ILTY. They're useful for everyday processing, between-session support, and while you're on a therapy waitlist. For diagnosed conditions (depression, anxiety disorders, OCD, PTSD, trauma), a licensed human therapist is the standard of care. For crisis, call or text 988 (US Suicide and Crisis Lifeline).

Did Woebot get removed from this list?

Yes. Woebot Health retired its consumer app on June 30, 2025. It was a major AI therapy app and for years would have been on a list like this. It's no longer available to consumers. See /best/woebot-alternatives-2026 for the dedicated Woebot-replacement roundup.

Are AI therapy apps safe during a mental health crisis?

No AI therapy app is a crisis intervention tool. Legitimate AI therapy apps (Wysa, ILTY) have crisis detection that directs users to emergency resources when distress signals appear, but the response is triage, not treatment. If you're in crisis, call or text 988 (US), or go to an emergency room. Use AI therapy apps for the everyday, not the acute.

How is this list different from your other AI app list?

Our /best/ai-mental-health-apps-2026 roundup is broader — it includes meditation, mood tracking, and peer support alongside AI therapy chatbots. This list is narrower, focused specifically on conversational AI functioning as therapy-style chatbots. If you want to compare Calm vs ILTY, use the broader list. If you want to compare Wysa vs Pi vs ILTY, use this one.

How often is this updated?
